夜针那天E2太低, 医生都没主意了,结果全部是优质胚胎,  有点奇怪.... [复制链接]

我取卵五个,成四个, 三个一级,一个二级的.

夜针那天医生也拿不定主意, 因为一个泡泡产生的E2是200-300之间. 结果我验血E2才280, 医生想多打一天促排,但我有三个泡泡都18了, 最后她决定打夜针算了.

我上次微刺激也是这样的情况, 夜针那天是320,取三个卵子, 得二个一级胚和一个三级胚, 移植失败.

我不知道是不是由于这项激素太低引起失败的........................

到处帮你找答案。英文能看懂吗？

at my u/s we had alot of follicles, but a very low E2 number. Could all the follies be just the pcos "ring of pearls" and be empty?

The u/s was done on CD8. I started with 175IU follistim and dropped to 125IU after 2 days. With luveris added every other day starting last Thursday. I had about 25 follies measuing 11-16mm and my lining was 9.2. But the E2 was only 131.

I take 5 units of lupron each morning

Todays u/s on CD10 showed the lead follies only 1-2mm larger, and more 10-11mm follies growing. E2 only went up to 178 and lining is 9.6 Dr wants me to add luveris every night instead of every other night and go back Friday to check again.

It sounds like you may have some ovarian resistance. Are you injecting the meds correctly? Please review your injection technique with your clinical coordinator. Otherwise, it may be a bit early and you may need more time.

update - -

We triggered Mon night when E2 only got to 675 even though there were 30 follies that looked the right size. Wed morning retrieval we only got 10 eggs. Todays report showed only 2 were mature and fertilized. We will do a 3 day transfer on Saturday if they make it. I am very upset with this. After 6 fresh IVF with this dr in 3 years, my results keep getting worse each time.

What protocol would help ovarian resistance? The estrogen priming protocol?

I hope it works for you this time. Estrogen priming if you have resistance or a step up protocol is used in cases like yours with many follicles on scan. Please contact me if you need to discuss the protocol options and your case overall.

well my cycle is over. we were going to do a 3 day transfer but my embies were only 2 and 3 cells and fragmented.

I am at a loss for how my results have changed so much in 3 years. Does egg quality go down that quickly? We had a perfect embie 3 years ago and now we get this bad?

It may get worse over time. Embryo qulaity is low if they only have 3-4 cells. Good luck.

Hi, Dr. Fisch, other docs,

I just called in today to find out that my e2 at trigger was around 467. I had 7 follies at the time, 5 of which were perfect in size. Dr. Fisch retreived 4, and 3 fertilized normally. My tx is tomorrow (Tues). My question is...should I be worried about such a low estrogen level as far as possible low embryo quality goes? I had poor embryo quality last time and had a low e2 level. I am on the LA8e2v protocol.

This time the low estrogen is appropriate for the number of follicles you have, so I would not be concerned.

Thanks for responding, Dr. Fisch. Wouldn't each follicle be about 200 of the e2? That would mean that 7 follicles times 200 equals 1400 e2? I know I had 3 good eggs--so would that then be what we count as far as e2? So many questions... Good thing I trust you, good Doc! I will see you tomorrow afternoon at 2:30pm ish.

Additionally, if you're using Antagon it can cause an artificial lowering of your estradiol level. In reality, it has recently been shown that these antagonists interfere with the test assay so that numbers are less useful than the trend.

英文看不太懂，你能大概说一下是什么意思吗？是不是也是免疫方面的问题？

有的医生说成熟一个卵子E2升高150-250，有的说200-300。上面的病人说B超照到7个卵子，可是打夜针时E2才467多，取卵取卵4个，配成3个，病人很担心胚胎的质量。一个医生回答说这个E2值是正常；另一医生说如果病人用来咭抗剂，E2会测出来会偏低，所以不能光看E2的数值，还要看B超的结果来判断卵子的成熟与否。
